At Autumn Leaves Nursing & Rehabilitation Center, our activity director carefully plans exciting and fun-filled events for everyone, whether it’s an old favorite or a new adventure. Activities such as socializing with friends, old and new, attending religious services, art classes, computer classes, gardening classes, and taking van trips enrich our residents’ quality of living. All of our activities are geared toward maintaining independence and a high quality of living for each resident.

Autumn Leaves Receives National Quality Award

The distinction is the second of three progressive award levels through the National Quality Award Program, which recognizes providers across the country that have demonstrated improved quality outcomes for staff and residents.
“Receiving the national Silver – Achievement in Quality Award is an honor and a testament to our team’s resident-centered approach to care,” said Jimmy Dale Zimmerman, administrator of Autumn Leaves Nursing & Rehabilitation Center. “This recognition reflects our staff’s commitment to providing care with compassion and excellence and we are proud to be recognized as a national leader in quality, safety and service.”
Implemented by AHCA/NCAL in 1996, the National Quality Award Program is centered on the core values and criteria of the Baldrige Performance Excellence Program, which is the foundation of the metric-based AHCA/NCAL Quality Initiative.
“Our mission here at Autumn Leaves is to improve quality of life,” said Thomas Little, assistant administrator of Autumn Leaves. “Our staff members take that responsibility seriously. Quality care is a huge part of achieving that mission and our team makes sure to deliver that care with compassion.”
Since 2018, Louisiana nursing facilities have received 137 national quality awards, and rank fifth in the nation for receiving the most Silver – Achievement in Quality awards.
“I commend the staff at Autumn Leaves for earning a national Silver – Achievement in Quality Award, a well-deserved recognition of their exceptional care, dedication to residents and unwavering commitment to continuous improvement,” said Louisiana Nursing Home Association Executive Director Wes Hataway.

Autumn Leaves, along with several other Central Management facilities, partnered with Dream Flights to honor our residents who bravely served our country with the adventure of a lifetime: a flight in a Boeing Stearman biplane, the same aircraft used to train many military aviators in the late thirties and early forties.
